Guests
To help with social distancing and to maintain the reduced number of people in our clinic at any given time, we ask that you limit any guests or caregivers accompanying you to your appointment.
Please reschedule your appointment if you answer “yes” to any of the following:
- Have you been in close contact with anyone with a confirmed case of Coronavirus/COVID-19 while they were ill in the last 10 days?
- Have you recently been tested positive for COVID-19 OR awaiting results? If yes, please reschedule your appointment after results are known.
- Are you experiencing a fever at or over 100.4 degrees?
- Are you experiencing a non-productive cough, not related to a chronic condition?
- Are you experiencing a new loss of taste or smell?
